Useful tips to help you prepare for your visit
Make a note
- Write down any mental health concerns, symptoms, or mood changes you want to discuss.
- Include when they began and what improves or worsens them.
What to bring
- List all current medications, supplements, and allergies (especially to medications).
- If you have any known allergies be sure to include those as well.
Health history
- Please bring your personal and family history of mental or physical health conditions, treatments, or hospitalizations.
Lifestyle
- Reflect on your daily routines and habits.
- Think about your sleep, stress, diet, exercise, and anything else that affects your mood or well-being.

Patient portal
In the patient portal, you can view and update most of your medical information including:
- Invoices / receipts billing
- Notes
- Appt scheduling
- Appt calendar
Follow ups & refills
- Follow up appointment: Schedule your follow-up in 2 weeks on the patient portal.
- Side effects: Contact us before your next visit if you notice side effects or behavior changes.
- Refill: For refills, call your pharmacy to confirm. If not scheduled, let us know & we’ll process it within 72 business hours.
